ABSTRACT

Objective: To explore the characteristics of neonatal adenoid development and to study the relationship between neonatal adenoid development and disease. Methods: A retrospective analysis of neonates who received an electronic rhinopharyngolaryngoscope at Shenzhen Children's Hospital from January 2019 to December 2020 was conducted to track the children's medical history and to analyze the adenoid development status. All 131 neonates successfully completed the electronic laryngoscopy. According to the presence or absence of visible adenoid hyperplasia, they were divided into a hyperplasia group (81 cases, 61.83%) and an un-hyperplasia group (50 cases, 38.17%). Results: Compared with the un-hyperplasia group, the age and birth weight of the adenoid hyperplasia group were larger, and the difference was statistically significant (Z age=-4.634,Z weight=-2.273,all P<0.05), but there was no significant difference in gender and gestational age between the two groups. The number of neonates with rhinitis/sinusitis in the hyperplasia group were significantly more than those in the un-hyperplasia group (62.96% vs 48%). Conclusion: The development of neonatal adenoids is related to daily age, birth weight, but not significantly related to gender and gestational age.

ABSTRACT

Objective: To compare the clinical characteristics and plasma inflammatory markers levels in different endotypes of chronic rhinosinusitis with nasal polyps (CRSwNP), and to explore the plasma biomarkers associated with endotypes of CRSwNP. Methods: A total of 74 CRSwNP patients (male/female: 41/33; average age: 40 years) and 40 control subjects underwent septoplasty in Tongji Hospital from January 2015 to December 2017 were enrolled in this study. The demographic and clinical features of all subjects including age, gender, past history, visual analogue scale (VAS) and CT scores were recorded. Patients with CRSwNP were divided into EoshighNeuhigh, EoshighNeulow, EoslowNeuhigh and EoslowNeulow four endotypes according to the eosinophil (Eos) percentage and neutrophil (Neu) count of nasal polyps tissue. Preoperative blood routine was performed and the levels of 27 biomarkers in plasma were measured by Bio-Plex suspension chip method. The clinical characteristics and the level of serum biomarkers of patients with different endotypes were compared. SPSS 18.0 software was used for statistical analysis. Results: There was no difference in the clinical features including gender ratio, age, course of disease, VAS score, endoscopy and CT score among EoshighNeuhigh, EoshighNeulow, EoslowNeuhigh and EoslowNeulow CRSwNP patients. Compared with EoslowNeuhigh and EoslowNeulow CRSwNP patients, patients with EoshighNeuhigh and EoshighNeulow endotype demonstrated a higher prevalence of atopy, allergic rhinitis and asthma comorbidity, and increased peripheral blood eosinophil absolute count and percentage (all P<0.05). However, there was no significant difference between EoshighNeuhigh and EoshighNeulow CRSwNP. Plasma levels of all 27 mediators including type 1 cytokines (IL-12 and IFN-γ), type 2 cytokines (IL-4, IL-5 and IL-13), type 3 cytokines (IL-17A), pro-inflammatory cytokines (IL-6 and TNF-α) and tissue remodeling-related markers (bFGF, VEGF and PDGF-BB) demonstrated no significant difference among all endotypes of CRSwNP (all P>0.05). Conclusions: Eoshigh and Eoslow CRSwNP patients display significant differences regarding the prevalence of atopy, allergic rhinitis and asthma comorbidity, peripheral blood eosinophil absolute count and percentage, but the clinical characteristics, blood cellular and biological markers can not effectively distinguish four endotypes of CRSwNP. Further studies are warranted to dig out the potential objective, convenient and reliable markers associated with endotypes in patients with CRSwNP.

ABSTRACT

Objective: To explore the types and clinical characteristics of chronic rhinosinusitis with nasal polyps (CRSwNP) based on artificial intelligence and whole-slide imaging (WSI), and to explore the consistency of the diagnostic criteria of the Japanese epidemiological survey of refractory eosinophilic chronic rhinosinusitis (JESREC) in Chinese CRSwNP patients. Methods: The data of 136 patients with CRSwNP (101 males and 35 females, aging 14 to 70 years) who underwent endoscopic sinus surgery from 2018 to 2019 in the Department of Otorhinolaryngology Head and Neck Surgery, the Third Affiliated Hospital of Sun Yat-sen University were analysed retrospectively. The preoperative clinical characteristics of patients were collected, such as visual analogue scale (VAS) of nasal symptoms, peripheral blood inflammatory cell count, total immunoglobulin E (IgE), Lund-Kennedy score and Lund-Mackay score. The proportion of inflammatory cells such as eosinophils, lymphocytes, plasma cells and neutrophils were calculated on the WSI of each patient through artificial intelligence chronic rhinosinusitis evaluation platform 2.0 (AICEP 2.0), and the specific type of nasal polyps was then obtained as eosinophilic CRSwNP (eCRSwNP) or non-eosinophilic CRSwNP (non-eCRSwNP). In addition, the JESREC diagnostic criteria was used to classify the nasal polyps, and the classification results were compared with the current gold standard for nasal polyps diagnosis (pathological diagnosis based on WSI). The accuracy, sensitivity and specificity of the diagnostic criteria of JESREC were evaluated. The data were expressed in M (Q1, Q3) and statistically analyzed by SPSS 17.0. Results: There was no significant difference between eCRSwNP and non-eCRSwNP in age distribution, gender, time of onset, total VAS score, Lund-Kennedy score or Lund-Mackay score. However, there was a significant difference in the ratio of nasal polyp inflammatory cells (eosinophils 40.5% (22.8%, 54.7%) vs 2.5% (1.0%, 5.3%), neutrophils 0.3% (0.1%, 0.7%) vs 1.3% (0.5%, 3.6%), lymphocytes 49.9% (39.3%, 65.9%) vs 82.0% (72.8%, 87.5%), plasma cells 5.1% (3.6%, 10.5%) vs 13.0% (7.4%, 16.3%), χ2 value was 9.91, 4.66, 8.28, 5.06, respectively, all P<0.05). In addition, eCRSwNP had a significantly higher level of proportion of allergic symptoms (nasal itching and sneezing), asthma, peripheral blood eosinophil and total IgE (all P<0.05). The overall accuracy, sensitivity and specificity of the JESREC diagnostic criteria was 74.3%, 81.3% and 64.3%, respectively. Conclusions: The eCRSwNP based on artificial intelligence and WSI has significant high level of allergic symptoms, asthma, peripheral blood eosinophils and total IgE, and the percentages of inflammatory cells in nasal polyps are different from that of non-eCRSwNP. The JESREC diagnostic criteria has good consistency in our research.

ABSTRACT

Objective: To investigate the difference of concentrations of specialized pro-resolving mediators (SPMs) derived from fatty acids in eosinophilic chronic rhinosinusitis with nasal polyps (ECRSwNP) and non-eosinophilic chronic rhinosinusitis with nasal polyps (nECRSwNP). Methods: A total of 36 patients with bilateral chronic rhinosinusitis with nasal polyps (CRSwNP) who underwent endoscopic nasal surgeries in Peking Union Medical College Hospital from May 2019 to September 2020 were enrolled, including 27 males and 9 females, with the age from 13 to 67 years. There were 23 cases of ECRSwNP and 13 cases of nECRSwNP. At the same time, 12 control subjects were enrolled. The concentrations of multiple SPMs, including lipoxins (LXA4 and LXB4), resolvins (RvD1, RvD2, RvD3, RvD5 and RvE1), protectins (PDX) and maresins (Mar-1) in nasal polyps with different histological subtypes and normal nasal mucosa were analyzed using liquid chromatography-tandem mass spectrometry. The concentrations of SPMs between groups were compared using Mann-Whitney U test. Spearman's rank correlation coefficient was used to measure the correlation between the concentrations of SPMs in nasal polyps and tissue eosinophil counts. Results: The concentrations of RvD2, RvD3, RvD5, LXA4, LXB4, Mar-1 and PDX in ECRSwNP group were significantly higher than that in controls (Z value was -2.276, -2.313, -3.371, -2.094, -2.051, -3.104 and -2.294, respectively, all P<0.05). The concentrations of RvD2, RvD5, Mar-1 and PDX in ECRSwNP group were significantly higher than those in nECRSwNP group (Z value was -2.175, -2.289, -2.243 and -2.124, respectively, all P<0.05). There was no significant difference in all these SPMs between nECRSwNP and controls (all P>0.05). The concentrations of RvD2, RvD3, RvD5, LXB4, Mar-1 and PDX in nasal polyps correlated positively with tissue eosinophil counts (r value was 0.443, 0.436, 0.371, 0.502, 0.340 and 0.386, respectively, all P<0.05). Conclusions: A varienty of SPMs are elevated in ECRSwNP. Dysregulation of fatty acid metabolism might play an important role in the chronic inflammation of ECRSwNP.

ABSTRACT

Objective: To identify the differentially expressed genes in nasal epithelial cells from chronic rhinosinusitis with nasal polyps (CRSwNP), and to analyze related genes which are involved in deficiency of nasal epithelial barrier in CRSwNP patients by analyzing the datasets download from the gene expression omnibus(GEO) database. Methods: The mRNA expression microarray data numbered GSE107624 (7 CRSwNP and 7 controls) and GSE69093 (13 CRSwNP and 11 controls) were downloaded from the publicly available GEO database. These two datasets were jointly analyzed to screen the differentially expressed genes in nasal epithelial cells of controls and CRSwNP patients. In the meanwhile, we further evaluated the function annotation and regulatory pathways of the differentially expressed genes. To further confirmed what we have observed, sinus tissues were collected from patients with CRSwNP (14 cases, 46.8±17.9 years) and uncinate process tissues were collected from patients with nasal septum deviation (7 cases, 23.4±2.3 years) as control group. The primary epithelial cells of nasal mucosa were cultured and the mRNA level of screened genes were measured by Q-PCR. SPSS 22.0 software was used to for statistical analysis. Results: GSE107624 dataset showed that there were 3 856 differentially genes in nasal epithelial cells between CRSwNP and control group, while there were 771 differentially expressed genes in GSE69093 dataset. Finally, 55 up-regulated genes and 3 down-regulated genes were noticed in nasal epithelial cells of CRSwNP patients in the two datasets. GO gene functional annotation analysis showed that SPTBN1, FNBP1L, VAPB and SNX1 were involved in cell adhesion function, MAP1B was participated in the formation of microtubule related complex. KEGG pathway enrichment analysis indicated that BAMBI and SIAH1 were involved in regulation of Wnt pathway, COL6A1 and EIF4E were involved in the regulation of PI3K-AKT pathway. String protein interaction network analysis assumed that MAP1B and VAPB were the core functional proteins. Among top 3 differentially expressed genes COL6A1, MAP1B and BAMBI, only MAP1B gene was increased in nasal epithelial cells of CRSwNP patients in comparison to controls. Conclusion: The increased MAP1B gene in epithelial cells of CRSwNP, as well as abnormal regulation of Wnt and PI3K-AKT signal pathways may mediate the barrier dysfunction in CRSwNP.

ABSTRACT

Objective: To investigate the roles of hypoxic stimulation in the pathogenesis of chronic rhinosinusitis with nasal polyps (CRSwNP) by comparing the variation and differences of inflammatory factors secreted from epithelial cells of nasal polyps and normal nasal mucosa under hypoxic stimulation. Methods: Sixty-eight patients who were diagnosed with CRSwNP from June 2015 to January 2018 at China-Japan Union Hospital of Jilin University were analyzed, including 36 males and 32 females, aged (45.2±12.5) years. Nasal polyps mucosa was included in CRS-NP group and inferior turbinate mucosa was included in CRS-IT group. According to the degree of eosinophil infiltration in histopathologic results, each of these two groups was further divided into eosinophil infiltration and non-eosinophil infiltration as Eos-NP group (n=34), Non-Eos-NP group (n=34), Eos-IT group (n=20) and Non-Eos-IT group (n=20). The inferior turbinate mucosa of twenty-five patients who were diagnosed with cyst of paranasal sinus or deviation of nasal septum was classified as control group (n=25), including 14 males and 11 females, aged (42.8±10.2) years. The expression of interleukin 17A (IL-17A), interferon γ (IFN-γ), tumor necrosis factor α (TNF-α) and hypoxia-inducible factor 1α (HIF-1α) in each group was analyzed by immunohistochemical staining. After 0, 24 and 48 h hypoxic stimulation, the secretion of IL-17A, IFN-γ, TNF-α in primary nasal mucosa epithelial cells of each group was tested by enzyme-linked immune sorbent assay (ELISA) experiment; the expression of HIF-1α was tested by immunofluorescent staining and imaging and Western blot. SPSS 17.0 software and two-way ANOVA were used for statistical analysis. Results: Immunohistochemical staining showed that the expression of IL-17A and TNF-α was much higher in control group (optical density (OD) value was 0.37±0.03, 0.53±0.02, respectively) and the expression of IFN-γ and HIF-1α was much higher in Eos-IT group (OD value was 0.47±0.03, 0.39±0.02, respectively). The secretion of IL-17A and TNF-α was much lower in control group than that in other groups under normal condition. After 48 h hypoxic stimulation, the secretion of IL-17A and TNF-α was much higher in control group compared with other groups. The secretion of IFN-γ in Eos-NP group was much higher than that in control group under normal condition ((13.7±1.3) pg/ml vs (11.1±1.6) pg/ml, P<0.05). After 48 h hypoxic stimulation, there was no difference of IFN-γ between control group and Eos-NP group. The expression of HIF-1α decreased in Eos-NP group and Non-Eos-NP group while increased in CRS-IT group and control group upon prolonged exposure to hypoxia. HIF-1α was mostly located at cytoplasm of epithelial cells in control and CRS-IT group while mainly located at nucleus of epithelial cells in CRS-NP group. Conclusions: The secretion of IL-17A, TNF-α, IFN-γ and the expression of HIF-1α show significant difference between normal nasal mucosa, polyps and inferior turbinate of CRSwNP under hypoxic stimulation, presenting different subcellular localization. This illustrates the proteins above are involved in transcription and regulation of the gene responsible for the pathogenesis of CRSwNP.

Introduction: For decades, animals have been used in sinonasal experimental models, and the practice has increased substantially in the last few years. This study aimed to assess the pathogenesis of infectious process and medication efficiency to treat rhinosinusitis. Objective: To evaluate the efficiency of the proposed experimental model to induce an acute bacterial sinonasal infectious process through histological analysis and sinus secretion cultures. Methods: This was an experimental study with 22 New Zealand rabbits, divided into: group A (six rabbits), group B (seven rabbits), group C (seven rabbits), and group D (control group with two rabbits). Rhinosinusitis was induced by the insertion of a synthetic sponge into the right nasal cavity of 20 animals (study groups), followed by the instillation of bacterial strains (50% Staphylococcus sp. and 50% Streptococcus sp.). The groups were euthanized within 10 days (group A), 17 days (group B), and 30 days (groups C and D). Results: All the rabbits of the study group developed acute bacterial rhinosinusitis, which was diagnosed through macroscopic evaluation, histological analysis, and sinus secretion culture. Conclusion: The proposed model is technically simple to perform, it is similar to the rhinogenic model in human beings, and it is highly efficient to reproduce an acute bacterial sinus infection. .


Introdução: A realização de modelos experimentais nasossinusais em animais vem sendo realizada há décadas, com substancial aumento nos últimos anos. Tem como objetivos identificar as alterações fisiopatológicas ocasionadas pelo processo infeccioso sinusal e avaliar a eficácia de medicamentos no tratamento da rinossinusite. Objetivo: Avaliar a eficácia do modelo experimental proposto para a indução de um processo infeccioso nasossinusal agudo bacteriano, utilizando parâmetros histopatológicos e cultura da secreção sinusal. Método: Estudo experimental com 22 coelhos da raça Nova Zelândia, divididos em: grupo A (6 coelhos), grupo B (7 coelhos), grupo C (7 coelhos) e grupo D (controle com 2 coelhos).Induzido quadro de rinossinusite através da inserção de esponja sintética nas fossas nasais direita dos 20 coelhos (grupos de estudo), seguido por instilação de toxoide bacteriano (50% estreptocócico, 50% estafilocócico). Os grupos foram sacrificados com 10 dias (grupo A), 17 dias (grupo B) e 30 dias (grupos C e D). Resultados: Todos os coelhos do grupo de estudo apresentaram quadro de rinossinusite aguda bacteriana, através da identificação macroscópica, análise histológica e cultura das secreções. Conclusão: O modelo proposto apresenta simplicidade técnica para sua execução, similaridade ao quadro rinogênico que acomete os humanos e é altamente eficaz na produção de um quadro infeccioso bacteriano agudo sinusal. .

ABSTRACT

PURPOSE: Evaluate and compare two different experimental techniques of maxillary sinus ostium occlusion using N-butyl cyanoacrylate in developing chronic histological findings without the inoculation of pathogenic bacteria among rabbits. METHODS: In a randomized study, sixteen New Zealand rabbits were assigned for occlusion of the right maxillary sinus through a transmaxillary approach or through the roof of the nasal cavity. The contralateral sinus served as a control. After 12 weeks, the animals were sacrificed for blinded histopathological analysis of the maxillary sinus mucosa. RESULTS: Histopathological changes consistent with CRS were found in eight (100%) of the maxillary sinuses approached transmaxillary and three of those through the roof of the nasal cavity (37.5%), p 0.008 and 0.250, respectively, comparing with the control side. Chronic mucosal changes were significantly better induced using the transmaxillary approach (p 0.026). CONCLUSION: It is possible to induce a model of chronic sinusitis among rabbits with transmaxillary sinus occlusion without bacterial inoculation. This model can be replicated for future cellular studies. .

Introducción: El estudio histopatológico de muestras obtenidas mediante cirugía endoscópica nasosinusal (CENS) es rutinaria en muchos centros hospitalarios, sin embargo, los reportes de sus resultados y el análisis de su real utilidad clínica son escasos en la literatura publicada. Objetivos: Describir y analizar el estudio de la anatomía patológica obtenida mediante CENS y conocer la correlación entre el diagnóstico clínico y la histopatología. Material y método: Se realizó un estudio descriptivo retrospectivo mediante revisión de fichas clínicas y recolección de datos anatomopatológicos, intervenciones por CENS en el Hospital Clínico Universidad de Chile. Resultados: El mayor porcentaje de los resultados histopatológicos fueron diferentes formas de rinosinusutis crónica (RSC) representando 65,67 por ciento (RSC simple, alérgica, poliposa alérgica y no alérgica). Entre los hallazgos con menor frecuencia destacan: papiloma invertido (4,9(0) por ciento), pólipo antrocoanal (9,31(0) por ciento), rinosinusitis fúngica no invasiva (2,45(0) por ciento) y 7 casos de patología maligna. Con respecto a la correlación clínica e histopatológica, se encontró para RSC una concordancia del 92,15 por ciento, para RSC poliposa del 95,94 por ciento y en pólipos antrocoanales 85,71 por ciento. Conclusiones: La mayoría de los resultados obtenidos fueron distintas formas de inflamación crónica de la mucosa nasosinusal. Se encontró una fuerte correlación entre el diagnóstico clínico y la histopatología, sin embargo, la aparición no excepcional de patología maligna inclina la conducta hacia el uso rutinario de solicitud de biopsia.


Introduction: Histopathological examination of samples obtained by endoscopic sinus surgery (ESS) is routine in many hospitals, however, results reports and effective analysis of its clinical utility are scarce in the literature. Aim: To describe and analyze the pathological study in ESS. Furthermore, recognize the correlation between histopathology and clinical diagnosis. Material and method: A cross-sectional study by reviewing medical records and collecting data from ESS interventions in the University of Chile Clinical Hospital was performed. Results: Most of the diagnoses were different forms of chronic rinosinusutis (CRS), representing 65.67 percent(simple, allergic and polypoid). Among the most common are unusual findings: inverted papilloma (4.9 percent), antrochoanal polyp (9.31 percent), fungal non-invasive rhinosinusitis (2.45 percent) and 7 cases of malignancy. Clinical and histopathologic correlation found for CRS was 92.15 percent 95.94 percent in nasal polyposis and 85.71percent in antrochoanal polyps. Conclusions: Most of the results were different forms of chronic inflammation of the sinus mucosa. A strong correlation between clinical diagnosis and histopathology was found, however, the onset of not exceptional malignant disease suggest the routine use of biopsy.

A histopatologia nasal de portadores do HTLV-1 com rinite crônica é desconhecida. OBJETIVO: Descrever aspectos histopatológicos de portadores do HTLV-1 com rinite crônica. CASUÍSTICA E MÉTODOS: Amostras de mucosa nasal de 10 portadores do HTLV-1 com rinite crônica, sendo oito com rinite alérgica e dois com rinite não alérgica, foram estudadas por microscopia de luz. Amostras de 10 pacientes com rinite alérgica não infectados pelo HTLV-1 serviram como controle. RESULTADOS: Fibrose subepitelial foi maior nos pacientes com rinite alérgica infectados pelo HTLV-1 (p=0,01), enquanto o espessamento da membrana basal foi maior nos controles (p=0,03). Houve tendência a menor eosinofilia e edema entre os infectados pelo HTLV-1, sem significância estatística (p=0,2). Para o infiltrado linfocítico, não houve diferença entre os pacientes com rinite alérgica infectados e não infectados (p=1,0). Fibrose subepitelial com infiltrado linfocítico de intensidade leve a moderada foram os achados encontrados nos dois portadores do HTLV-1 com rinite não alérgica. CONCLUSÕES: O estudo sugere que a infecção pelo HTLV-1 pode modificar a histopatologia da rinite alérgica, sobretudo por maior fibrose, e pode estar relacionada a uma rinite crônica não alérgica com infiltrado linfocítico.


The nasal histopathology of HTLV-1 carriers with chronic rhinitis is unknown. OBJECTIVE: To describe the histopathological features of HTLV-1 carriers with chronic rhinitis. MATERIALS AND METHODS: Biopsies of nasal mucosa of ten HTLV-1 carriers with chronic rhinitis (eight patients with allergic rhinitis and two patients with non-allergic rhinitis) were studied using a light microscope. Samples from ten patients with allergic rhinitis not infected with HTLV-1 were used as controls. RESULTS: Subepithelial fibrosis was more pronounced in patients with allergic rhinitis infected with HTLV-1 (p=0.01), while the basement membrane thickness was greater in controls (p=0.03). There was a trend towards less eosinophilia and edema among those infected with HTLV-1, without statistical significance (p=0.2). For the lymphocytic infiltrate, there was no difference between infected and not infected patients with allergic rhinitis (p=1.0). Subepithelial fibrosis associated to moderate or small number of lymphocytes were found in the two HTLV-1 carriers with non-allergic rhinitis. CONCLUSIONS: This study suggests HTLV-1 may modify the histopathology of allergic rhinitis, especially by promoting subepithelial fibrosis, and may be related to chronic non-allergic rhinitis with lymphocytic infiltrate.

ABSTRACT

A rinossinusite é uma doença frequente na prática diária de clínicos gerais, pediatras, alergologistas, pneumologistas e otorrinolaringologistas. Embora tenha baixa mortalidade e alta morbidade, há situações específicas que precisam ser reconhecidas, em que as complicações podem colocar a vida do paciente em risco. Por ser abordada por diversas especialidades e ter formas variadas de apresentações, é importante definir uma classificação comum a fim de estabelecer diagnósticos mais precisos, racionalizar a solicitação de exames complementares e instituir tratamentos adequados. Dessa forma, haverá uma redução nos custos do tratamento, nas complicações e nas formas crônicas da doença, garantindo uma melhor qualidade de vida para o paciente.

ABSTRACT

To compare the efficacy of topical nasal steroids therapy with total inferior turbinectomy in relieving nasal obstruction caused by inferior turbinate hypertrophy. Randomized Controlled Trials [RCT]. The study was conducted in the ENT department, Combined Military Hospital Rawalpindi from October 2006 to July 2007. Sixty patients suffering from allergic rhinitis were divided in Two groups of thirty partients. Group A underwent total inferior turbinectomy where as Group B advised Betamethasone nasal drops for two months. Relief in terms of nasal obstruction was assessed on Visual analogue scale two months after commencement of treatment. Chi-square test was applied as test of statistical significance to compare the two groups. Total inferior turbinectomy was found to be clinically superior to topical betamethasone drops in relieving nasal obstruction caused by inferior turbinate hypertrophy [p<0.001]. Total inferior turbinectomy is a better method of relieving obstruction as compared to topical betamethasone drops in nasal obstruction due to hypertrophied inferior turbinates

ABSTRACT

A criação de modelos de estudo das rinossinusites é necessária para compreensão de seus aspectos fisiopatológicos. OBJETIVO: Sugerimos neste estudo estabelecer um modelo experimental fidedigno, reproduzível e consistente para a rinossinusite inflamatória sem uso de inoculação de agentes infecciosos em coelhos. Tipo de Estudo: Experimental. MATERIAL E MÉTODO: Foram utilizados 20 coelhos neste estudo. Os animais foram submetidos a 4 intervenções diferentes: colocação de tampão de esponja unilateral, obliteração unilateral de óstio nasal com cianoacrilato, instilação unilateral de antígenos em seio maxilar e instilação de sangue em cavidade maxilar unilateral. Os animais foram acompanhados até 15 dias do início do estudo e ao final do período de seguimento foram anestesiados e sacrificados. Os seios maxilares foram avaliados histologicamente e os resultados comparados com os seios maxilares contralaterais para controle e entre os grupos de intervenção. RESULTADOS: Todos os animais do estudo desenvolveram rinorréia amarelada unilateral até o 15º dia de acompanhamento. Apenas os animais que receberam sangue não apresentaram alterações histológicas compatíveis com rinossinusite purulenta. CONCLUSÕES: O uso de esponja e cola para obstrução do óstio meatal e a aplicação de toxóide são eficientes como produtores de modelo de rinossinusite. A aplicação de sangue no antro não produz os mesmos resultados.


Experimental models for clinical studies of rhinosinusitis are needed. AIM: to define a reliable, solid and reproducible experimental model for inflammatory rhinosinusitis with no innoculation of infectious agents in rabbits. Study design: Experimental. MATERIAL AND METHOD: Twenty 20 rabbits were divided into 4 groups submitted to 4 different interventions: the placement of a unilateral nasal fossa sponge, unilateral obliteration of the nasal ostium with cyanoacrylate, unilateral placement of antigens in the maxillary cavity and unilateral placement of blood in the maxillary cavity. The animals were monitored for 15 days and then anesthetized and sacrificed; the maxillary sinuses were evaluated histologicaly and results were compared with controls and between the intervention groups. CONCLUSION: Sponje and glue as agents of meatal obstruction and toxoid aplied in the antrum are efficient as methods for rhinosinusitis modeling. Blood was not efficient in producing sinusitis.

ABSTRACT

The purpose of this study was to compare the paranasal sinus mucosal thickenings, bony changes consistent with chronic sinusitis, and bony anatomic variations detected by computed tomography (CT) in chronic rhinosinusitis patients with and without allergy. Three hundred and thirty-nine patients with chronic rhinosinusitis were analyzed for their allergic status by performing skin prick test. Two hundred and thirteen patients (62.8%) had at least one positive skin prick test (allergic patients, male/female: 85/128, mean age: 29.1 +/- 1.2). One hundred and twenty-six patients (37.2%) were included in the non-allergic group (male/female: 53/73, mean age: 31 +/- 2.2). Maxillary mucosal thickening and frontal hypoplasy were significantly more common in allergic chronic rhinosinusitis patients. Moreover, pneumatized uncinate process is apparently more common in the allergic group than non-allergic group, and statistical analysis revealed marginal significance (p = 0.0535). In conclusion CT findings of allergic chronic rhinosinusitis patients are comparable to the CT findings of chronic rhinosinusitis patients without allergy. However, presence of maxillary mucosal thickening, frontal hypoplasia or pneumatized uncinate process in the CT scan of a patient with chronic rhinosinusitis could be of clinical significance, and might guide the otolaryngologist for the evaluation of the presence of allergy.

ABSTRACT

Eosinophil and mast cell infiltrations are consistent findings in nasal polyp tissue. Previous studies have shown that matrix metalloproteinases (MMPs) may be involved in eosinophil infiltration in airway mucosa of asthmatic patients, and that transforming growth factor-beta1 (TGF-beta1) induces extracellular matrix deposition in nasal polyp tissue. The aim of this study was to evaluate the role of MMPs and tissue-inhibitor of metalloproteinase-1 (TIMP-1) in association with TGF-beta1, eosinophils and mast cell activation in nasal polyp tissue. Nasal polyp tissues from 20 patients who underwent polypectomies were collected and prepared into tissue homogenate. Eosinophil cationic protein (ECP) and tryptase levels were measured by CAP system (Pharmacia, Sweden). MMP-2, MMP-9, TIMP-1 and TGF-beta1 levels were measured by enzyme-liked immunosorbent assay. MMP-2 was the predominant form of MMPs, followed by MMP-9 and TIMP-1. There were significant correlations between ECP, and MMP-9, MMP-2, TGF-beta1 and tryptase, but not with TIMP-1. Significant correlations were noted between tryptase, and MMP-2, MMP-9, and TGF-beta1, but not with TIMP-1. Close correlations were noted between TGF-beta1, and MMP-9 and MMP-2, but not with TIMP-1. MMP-2, MMP-9, and TGF-beta1 may contribute to eosinophil and mast cell migrations into nasal polyp tissue.

ABSTRACT

Objetivo: determinar si la exposición de niños sanos a diferentes concentraciones de contaminación atmosférica altera morfológicamente el epitelio respiratorio nasal. Material y método: se incluyeron 71 niños sanos que se dividieron en tres grupos: Grupo I, 31 niños originarios de La Ciudad de México, Grupo II, 21 niños originarios de Tula Hidalgo, un área moderadamente contaminada, y Grupo III, 19 niños originarios de Contepec, Michoacán, una zona rural no contaminada. A todos los niños se les tomó biopsia nasal del cornete inferior y se les realizó citología nasal. Las muestras se examinaron dos veces por el mismo patólogo con una técnica doble ciego. Se tomaron mediciones de ozono durante 1992 en la Ciudad de México. Resultados: se presentaron síntomas nasales (obstrucción, prurito y sequedad nasal) en 96.8, 71.4 y 52.6 por ciento de pacientes en los grupos I, II, III, respectivamente (p = 0.001). También se encontraron cornetes hipertróficos bilaterales en el 100 por ciento de los niños en el grupo I contra 81 y 73 por ciento en los grupos II y III (p = 0.001). El examen citológico no reveló diferencias significativas entre los grupos estudiados, aunque los valores de p fueron de 0.06 con respecto a la inflamación y a las células caliciformes anormales cuando se comparó el grupo I control los grupos II y III. Con respecto al examen histológico se encontraron diferencias significativas cuando se compararon el grupo I contra los grupos II y III en relación con metaplasia escamosa (p =0.002) e hiperplasia pseudoepiteliomatosa (p = 0.003), sin encontrar diferencias significativas en la queratización (p=0.06) y displasia (p=0.06). Conclusión: los resultados de este estudio indican que la exposición a una atmósfera contaminada es seguida por cambios agudos en la histología y citología de la mucosa nasal en niños sanos, siendo estos cambios más frecuentes y agudos en las áreas más contaminadas. Estos hallazgos implican que deben ponerse más atención en los efectos a largo plazo de la contaminación atmósferica a fin de establecer medidas de control más efectivas

As rinites e rinssinusites estäo entre as patologias mais frequentes na prática clínica. Embora pouco valorizadas, trazem grande desconforto e podem associar-se a problemas graves, tais como: apnéia do sono, asma perene refratária ao tratamento e infecçöes respiratórias repetidas. Contribuem de forma sinérgica para a patogênese das rinites: predisposiçäo genética, aspiraçäo de alégenos, de agentes irritantes, viroses respiratórias e problemas psicossociais. Acredita-se hoje, em analogia ao observado na asma, que as rinites, quer sejam elas alérgicas ou näo, resultam de processo inflamatório na mucosa nasal, que é o substrato da hiper-reatividade encontrada. O resfriado comum é a causa mais frequente da rinite aguda. Entre as rinites crônicas, destacam-se pela maior frequência a rinite alérgica, a rinite näo-alérgica com eosinofilia e a rinite vasomotora. O diagnóstico diferencial das rinites crônicas baseia-se fundamentalmente na história e é confirmado com os achados de exame físico, testes alérgicos e citologia nasal. A rinite alérgica tem história característica, eosionfilia e testes cutâneos positivos a aeroalérgenos comuns. A rinite näo-alérgica eosinofílica diferencia-se da rinite pela negatividade dos testes cutâneos. A rinite vasomotora é um diagnóstico de exclusäo. Como recursos para o tratamento sintomático, temos os descongestionantes para alívio da obstruçäo e o brometo de ipratrópio, que pode ser útil para a rinorréia persistente. No tratamento da rinite alérgica, o mais importante säo as medidas de controle ambiental, principalmente o combate ao ácaro da poeira domiciliar, agente mais comumente envolvido na alergia respiratória. Os anti-histamínicos inibem o prurido e os espirros. Quando os sintomas säo muito frequentes, deve-se lançar mäo também do cromoglicato dissódico ou de corticosteróide tópico, que têm a capacidade de reduzir o processo inflamatório e consequentemente a hiper-reatividade nasal, enquanto se aguardam os benefícios de medidas preventivas. O cromoglicato é eficaz apenas em quadros leves a moderados de rinite alérgica. Os corticosteróides tópicos säo a opçäo terapêutica mais potente para a rinite alérgica grave e, habitualmente, também trazem benefícios na rinite näo-alérgica com eosinofilia
